If you have developed a truly person centre service, introduced a quality initiative, or delivered excellent care that has made a difference to the people that you care for, please be 'proud' and tell us about it.

To do this you can:

• complete a nomination form for our Proud to Care Awards:
  • application form, with guidance included [Word]
  • consent form for applications including photos or films - managers [Word]
  • consent form for individuals who are featured in photos or films included in the application [Word]
